Please review these instructions before starting your application.
Course Requirements
Supervision courses must include an academic component to receive course credit. Completing internship, placement, or extracurricular hours alone is not sufficient. Please check with your Instructor before applying to make sure you have the correct course information and requirements.
Internship Students
If you are completing an internship, please have your Site Supervisor’s approval document ready before starting this form.
Acceptable documents include:
- A PDF of an email showing your Site Supervisor’s approval
- A signed approval form
- Other written confirmation of approval
The document should clearly show your Site Supervisor’s name, approval, and relevant internship information.

You must sign in with your CSUSB Microsoft account to access this form.
Enter your account as:
CoyoteID@csusb.edu
Example: 001234567@csusb.edu
Then click Next and sign in with your CSUSB password.
Important: Use your Coyote ID followed by @csusb.edu on the Microsoft sign-in screen. Do not use your personal email address or your @coyote.csusb.edu email address.
