The following curriculum forms are in MS Word. Please download forms and save to your computer before entering information. Completed forms, including all signatures, must be submitted to the Academic Curriculum and Scheduling Office in AD-163 no later than 10 business days prior to scheduled University Curriculum Committee meetings. UCC meetings take place in late May, October, November and January. Forms submitted after the January UCC meeting will not be reviewed until May and are considered for the following Academic Year cycle.

Justification is completed on the Academic Programs Form GE - General Education Form (Addendum to Academic Programs Form C) and is not needed on the Academic Programs Program Form P – Change in General Education Program.
