University Police Department Alcohol Use Permit

This form must be completed in its entirety and submitted to CSUSB Special Events and Guest Services at least ten (10) working days prior to the event. Special Events and Guest Services will review and submit this form to the University Police.  Additional County and State permits maybe be required.

By submitting this form, I acknowledge that I have read the California State University, San Bernardino policy on serving alcoholic beverages. I agree to comply with the provisions of said policy and recognize that any violations may result in civil or criminal penalties and/or University-imposed sanctions. The Requestor (named below) must be a full-time CSUSB management employee/administrator, or his/her designee, or faculty member or lessee. This individual must assume responsibility for conduct at the event and be present during the entire period in which the alcohol is consumed.
