Below are forms and documents and a description of why a student may need this form.

Advising Release Form

Students with administration advising holds should complete the JHBC Online Advising Release Form, save, and email to the Student Success Center for review.

 

Permission Requests

Use the new online feature Permission Requests for classes that are full (closed), have time conflicts (simultaneous enrollments), will exceed max enrollment units (course overloads), and/or have class requisites (prerequisites, student standing, major, etc). Review Permission Request instructions.

Course (Unit) Overload 

An overload permit is required for any student wishing to exceed the maximum amount of units allowed during the priority and open registration period.

Requests accepted only during the Late Registration period in a term. If the class you are requesting to overload is waitlisted, submit a unit load Permission Request after the waitlist period ends.

Once Late Registration begins, use the Permission Request link in the Student Center to submit a request.

Overload requests cannot be submitted until the second week of classes - check the academic calendar for dates.

Petition to Drop After Census Date

A Permission Request must be submitted online via the Student Center in order to drop a class after the Census Date of each term. Approval will only be granted for serious or compelling reasons, and documentation is required. The Permission Request will be sent to the instructor, then the department chair of the class, and finally to the College of the student's major (JHBC Dean's designee in the JHBC Student Success Center).

Poor performance or attendance is not considered a serious or compelling reason, and petitions citing this reason will be denied. If the petition is approved the course is defined as a withdrawal and a grade of 'W' will be posted on the student's record.

Petition to Add a Course after Census Date

A Permission Request must be submitted online via the Student Center in order to add a class after the Census Date of each term. Graduation Requirement Check

Undergraduate Students

Students must request a graduation requirement check (grad check) online prior to the end of the term of their expected graduation. Review the Graduation Requirement Check instructions. However, to avoid late fees, the grad check should be filed by the established deadlines listed below. Students should not request the grad check until they have accumulated 135 quarter units toward their degree. This may include work in progress.
