If an accident occurs, please contact the Risk Management Office first. Our team is trained to assess, respond, and support all safety-related matters. Prompt reporting helps us not only respond effectively but also identify patterns and prevent future incidents.
We encourage everyone whether you're a student attending class, a faculty member, a volunteer, or a visitor to stay informed and involved in keeping our campus safe.
Safety Brochure
Drivers representing the university are required to report all vehicle accidents that occur while driving State-owned vehicles as well as those occurring in personal or rental vehicles when driving on university business.
1. Check that all passengers (including yourself) are OK
Move far away from the road, but remain at the accident scene. Warn oncoming traffic by activating your hazard lights and/or setting flares if you have them.
2. Call 911 to report the accident
For non-emergency campus accidents, call University Police at (909) 537-7777.
3. Do not admit fault
To protect yourself legally, do not discuss the accident with anyone other than the police, your supervisor, authorized State officers, Risk Management, and your insurance company.
4. Exchange information
If you are driving a rental or private vehicle, use the information form on the left insert of this brochure as a guide. State-vehicle drivers should complete STD Form 269 before leaving the scene of the accident.
5. Notify your supervisor
Immediately report the accident to your supervisor. Additionally, complete STD Form 270 within 24 hours of the incident and send to riskmanagement@csusb.edu. Supervisors will need to complete STD Form 274, your supervisor may contact you for additional information to complete the form.
6. Contact your insurance company
Drivers of private or rental vehicles should contact their insurance to open a claim.
Tow Truck Information
To arrange towing for a state vehicle, you must first contact the Grounds and Fleet Manager or their designee in Facilities Planning and Management (FPM) for authorization. For after-hours, please call University Police Department at 909-537-7777 to be connected with the FPM manager on duty. Please note that unauthorized towing may lead to you being responsible for the towing costs.

If you are driving a rental vehicle and are involved in an accident, follow these steps:
1. Report the Loss Immediately
Always carry your rental documents with you
If an issue arises, contact the rental car agency immediately. You can find their contact details on your rental agreement. Avoid making any promises or commitments without fully understanding the situation.
2. Present your Coyote ID (physical card preferred)
Your ID must be shown at the time of rental to ensure coverage under state-negotiated contracts.
3. Contact the CSU System Wide Office of Risk Management on the same calendar day (including Friday, Saturday, Sunday)
All accident claims must be referred directly to: (916) 376-5302 or (800) 900-3634
They manage all claims with rental car agencies.
4. Notify the CSUSB Risk Management Office within the same day or by the next business day
Report any loss to your campus risk manager for proper documentation and support.
Note: No additional insurance is required when using approved rental contracts. Coverage includes up to $500,000 per accident.
