Dean's Approval to Repeat a Course Form

This request for a course repeat will need to be completed by you for the following 2 reasons:

  1. You have taken a class 2 times and either failed or received an N/C each time.  You need permission to take it a 3rd time.
  2. You have taken the class once and received a “C” or better and want to retake the class for a higher grade.

The Dean's Approval to Repeat a Course can now be requested through your MyCoyote Student Center. Please review the instructions below.

Request Permission to Refile a Grad Check

In order to refile your grad check, please complete the following two steps:

Step 1: Refile a Grad Check Request

Step 2: If approved, please refile your grad check online via MyCoyote's Student Center.

Students Submitting an Impacted Change of Major/Minor

Step 1: Students seeking to change or add into an Impacted Major or Minor must first seek approval and signature from the Department Chair.

  • Criminal Justice (Andrea Schoepfer, Department Chair)
  • Psychology (Christina Hassija, Interim Department Chair)
    • An Academic Advisor must review all student requests to declare Psychology as a minor before receiving the department chair's signature. Students should contact to initiate the review process.

Step 2: Student's approved request must be submitted to Office of the Registrar ( and include the following:

  • Why are you making the request?
  • Expected term and year of graduation
  • Attached PDF form

Petition for a Retroactive Add

This form must be completed when a student needs to add a class to a past term.
