ALL persons who travel to a foreign country on CSUSB related business/activities (regardless of funding source) are required to submit a fully completed and authorized Travel Survey which will inform the insurance and approval of our application.

Prior to making your travel plans, check each list for the warnings of the country you plan to visit:
If your destination appears on either list, you must submit your Travel Survey to Risk Management at least 30 days in advance of your departure to provide enough time for underwriting and risk assessments.
Please Note: As of January 2015, you need to obtain President approval for travel to countries that appear on the warning lists above. Insurance requests are sent to the Chancellor's Office for processing and it requires underwriting by a third-party insurance so it is important to plan in advance.
The Risk Management Office needs this advance notice of your international travel in order to secure coverage for you in CSURMA's foreign travel insurance program offered through ACE and to conduct a pandemic risk assessment. Foreign insurance provides medical and other important coverage in the event of an emergency situation. Insurance does not cover COVID-19 disruption. Additional travel and lodging expenses are the responsibility of the sponsoring department and the risk assessment provides a snapshot of potential pandemic challenges and mitigation options.
If we do not receive your travel authorization form well in advance of your travel, we may not be able to secure coverage for you at all. Please note that your travel will be denied if the form is not received by the Risk Management Office with enough time to obtain insurance coverage for you.
Further, it is recommended that all employees traveling internationally sign up for the State Department's Safe Traveler Enrollment Program (STEP).
