When submitting a request form or application, you will need to bring the form to the Office of the Registrar for proper screening of fees, major and/or academic holds. Since all fees are submitted to the Bursar's Office for payment, you will be asked to take the form downstairs to the Bursar's Office (UH-035) to complete your transaction. Picture ID is required.
If you are mailing your request or application, please make check or money order payable to CSUSB. Or you may pay for fees over CashNet, print a copy of your receipt, attach it to the form, then mail to the Office of the Registrar. Any questions regarding a request form or application will be addressed specifically with the student.
Please print, complete request form or application and return to the Office of the Registrar (UH-171) either in person, with picture ID, or by mail to:
