Before you drop off exams (Form NCS 30423), please ensure that:

Text Scoring Request PDF Format
- All Students' Names (on test sheets) are bubbled in under the NAME section of each sheet and labeled as shown in this picture.
- All Students' Identification (on test sheets) are bubbled in under the Identification Number section of each sheet and labeled as shown in this picture.
- All Students' Grade or Education (on test sheets) are bubbled in under the Grade or Education section of each sheet and labeled as shown in this picture. (It should be 1 for version 1, 2 for version 2, etc.) versions.)
For the Answer Key sheet:
- Name Section is bubbled and spelled out with ANSWER KEY.
- Identification Number section is bubbled in with Zeros (000000000)
Retention Schedule
Faculty who wish to retain original exam materials, including answer keys, student responses, and hard-copy grading results should claim them within 30 days of grading. Any unclaimed materials are destroyed 30 calendar days after grading results are delivered.
Materials can be claimed from the Technology Support Center (TSC) (PL-1108) Monday to Thursday from 7:00 am to 10:00 pm and Friday and Saturday from 7:00 am to 7:00 pm.
