Incomplete Extension

One year must have elapsed since course was originally taken. An incomplete extension less than one year only requires a memo to be sent from the instructor to the Office of the Registrar.

Print the following:

  1. Name

  2. Address, City, State and Zip Code 

  3. Telephone

  4. Coyote ID

  5. Major 

  6. E-mail

  7. Mark Extension of Time to Remove an Incomplete 

  8. Include Course (i.e. Spanish 514)

  9. Term Course was taken (Fall, Winter, Spring, Summer) 

  10. Year Course was taken (i.e. 2004)

  11. Extended until (Specific Date - i.e. January 4, 2005)

  12. Take form to the instructor of the Course 

  13. Have instructor print, sign Name and Date 

  14. Include department  (i.e. Spanish)

  15. Include Reason for Your Request 

  16. Sign and Date petition 

Steps to Completing Petition for Other Reasons:

If you would like to submit a petition for other common requests such as the following:

  • Waiver of 90-unit minimum to take Writing requirement (306 course - English department chair must approve)

  • Post-Baccalaureate course credit/GPA to count as undergraduate credit/GPA

Print the following:

  1. Name

  7. Mark Other 

  8. Specifically indicate what you are trying to petition 

  9. Include reason for your request 

  10. Sign and Date petition
