Mild-Moderate Level I/Getting Started

In addition to meeting the general requirements of the university for admission as a postbaccalaureate student, admission to the Preliminary Level I Education Specialist Instruction Credential: Mild/Moderate Disabilities program requires you submit the following:

Graduate Application to the university Graduate Application. Submit a completed graduate application. Available from Admissions and Student Recruitment - (909) 537-5188 or on the web at http://www.csumentor.edu/AdmissionApp.

Application for Admission to the College of Education

Letter documenting a minimum of 45 hours in a K-12 classroom

Original verification of passing or failing scores on the CBEST - California Basic Educational Skills Test Note: Passing scores required for recommendation for the credential.  See the CBEST Website.

Negative tuberculin examination within the last four years. See the CASE web page "TB Information"

Subject matter competence examination passage or coursework - See the CSET/SSAT website

 

Certificate of Clearance (fingerprinting) or evidence of a credential or permit authorizing public school teaching in California; and fingerprinting. See the CASE web page "Fingerprinting"

Group Advising Session attendance sheet - see "Group Advising Meeting" schedule.

Advisor Confirmation Form demonstrating meeting between you and your assigned advisor. Complete coursework plan draft before meeting with advisor. See "Coursework Plan"
