Listed below is a description of the services the Credential Processing Office provides to assist you with the process of your application for formal credential/certificate recommendation to the Commission on Teacher Credentialing (C.T.C.). Application for Formal Certificate of Clearance

The Certificate of Clearance verifies that the credential candidate has completed the C.T.C. professional conduct review evaluated by the Department of Justice and the Federal Bureau of Investigation. Credential candidates who do not hold a valid California credential or have not previously applied for a Certificate of Clearance through C.T.C. must submit an Application for Certificate of Clearance Recommendation prior to official and unconditional admission to a credential program or enrolling in course work requiring field experience in public schools.

NOTE: Credential candidates must submit the Certificate of Clearance application no less than 60 working days prior to admission to student teaching, supervision or fieldwork.

How to apply:

  1. Request for Live Scan Service (41-LS) form – Complete the applicable sections of the form prior to arriving at the Live Scan site offering electronic submission of fingerprint services. Applicants may refer to the CSUSB Police Department website for applicable fees and hours of operation or call (909) 537-3552 to schedule an appointment for Live Scan service. A current listing of other Live Scan sites offering electronic fingerprint services is available on the California Attorney General’s website or you may contact your local police or sheriff station for Live Scan service availability and hours of operation.
  2. Certificate of Clearance Student Application – Complete this application electronically on the C.T.C. website and pay the $29.50 ($27.50 application fee + $2 service charge) application fee. NOTE: A Visa or MasterCard debit or credit card will be required for payment transaction.
  3. Confirmation Page - C.T.C. will immediately e-mail the applicant a confirmation number and direct them to a website to track the payment after step 2 has been completed. Within 1 – 3 working days thereafter, the status of the Certificate of Clearance will be available to view on the C.T.C. Application Status and Credentials Held look-up site. NOTE: If C.T.C. requires additional documentation, the Certificate of Clearance process will be delayed.

Effective January 1, 2006, C.T.C. will no longer mail the applicant the Certificate of Clearance document.  Therefore, the Certificate of Clearance must be recorded as “issued” on the C.T.C. Application Status and Credentials Held look-up site in order to be officially and unconditionally admitted to a credential program or enrolling in course work requiring field experience in public schools.

Per C.T.C. (Title 5 sections 80028(b) and 80487(b)), applicants with a Certificate of Clearance issuance date of January 1, 2007 and after must use their Certificate of Clearance credit towards the issuance of the their initial credential only. If an applicant does not use the Certificate of Clearance credit for the initial credential application, the credit will be deemed earned upon receipt and will not be refunded. Certificate of Clearance credit issued prior to January 1, 2007, must be used towards the next credential application.

Request for Pre-evaluation

The purpose of the Request for Pre-evaluation is to determine the status of your credential/certificate program and eligibility to apply for formal recommendation of the credential/certificate. The Request for Pre-evaluation is initiated with the submission of the Request for Pre-evaluation form one quarter prior to the last quarter of program course work. Please note that there is no fee associated with the Request for Pre-evaluation and that there will only be one Pre-evaluation conducted for each type of credential/certificate program. COMPLETE REQUEST FORM NOW

Pre-evaluations will not be conducted for:

  • Candidates requesting Internship eligibility. Internship candidates will need to contact the Internship program coordinator regarding their program status. Internship credential holders can submit a Request for Pre-evaluation one quarter prior to the last quarter of program course work.
  • Designated Subjects (Adult, Vocational, Career Technical Education, Special Subjects and Supervision and Coordination) applicants. Applicants will continue to submit the Designated Subjects credential application via Paper Application for Credential Recommendation in person or by mail.

Although the original Required Documentation is not necessary for this request, it will be required for the submission of the application for formal credential/certificate recommendation. It is the applicant’s responsibility to verify that all credential/certificate requirements are successfully completed and that all required original documentation is in their program file prior to or at the time of submission of the application for formal credential/certificate recommendation. Please note that all candidates must be officially and unconditionally admitted to the University and to the appropriate credential/certificate program in order to be formally recommended for a credential/certificate by CSUSB.

The results of the Pre-evaluation will be e-mailed (via attachment) to the applicant's CSUSB e-mail address only or mailed (per request or if applicant does not have a valid CSUSB e-mail address) within approximately 2 - 4 weeks.

Application for Formal Credential/Certificate Recommendation

Effective April 1, 2007, C.T.C. no longer accepts paper applications for formal credential/certificate recommendation (except for Designated Subjects applications). All other credential/certificate recommendations must be processed via on-line by the recommending institution. NOTE: Designated Subjects (Vocational/Career Technical Education/Adult/Special Subjects/Supervision and Coordination) credential applicants need to process Paper Application for Designated Subjects Credential Recommendation (see instructions below).

Effective September 1, 2008, C.T.C. will no longer print and mail the credential/certificate documents however, these documents will be viewable and available for printing on the C.T.C. Application Status and Credentials Held look-up site within 48 hours of issuance.

This application initiates a program evaluation and recommendation to C.T.C. for credential/certificate issuance. The application may be submitted at the beginning of the last quarter of program course work and after verification of successful completion of all original Required Documentation has been submitted. Please note that incomplete applications will be returned to the applicant for completion and resubmission.

The results of the program evaluation will be e-mailed (via attachment) to the applicant's CSUSB e-mail address only or mailed (per request or if applicant does not have a valid CSUSB e-mail address) within approximately 2 - 4 weeks.

NOTE: The recommended credential issuance date will be the official program completion date (includes course work, CPR, and/or examination(s), if applicable) -OR- if three months have passed since the official program completion date, the recommended credential issuance date will be the date the completed application was accepted by the Credential Processing Office.

Application for On-line Credential Recommendation

This process allows for the electronic processing of the credential/certificate recommendation by CSUSB as well as the electronic submission of the on-line Student Application to C.T.C.  COMPLETE APPLICATION NOW

The On-Line Application for Credential Recommendation is initiated with the submission of the Application for On-Line Credential Recommendation, Required Documentation and the *$25 CSUSB non-refundable processing fee (click Payment Methods for further details). Please note that the use of this process will require a Visa or MasterCard debit/credit card to pay the C.T.C. on-line application fee and a permanent e-mail address (valid at least 3 months from the submission date of application) to receive e-mail correspondence directly from C.T.C. NOTE: Applicant must have a valid CSUSB e-mail address (click CSUSB Telecommunications and Network Services Web Mail for further details) in order to receive e-mail correspondence from the Credential Processing Office regarding the application status. If applicant does not have a valid CSUSB e-mail address, correspondence will be mailed to the address listed on the application.

Once the credential/certificate recommendation has been submitted to C.T.C., within a couple of days thereafter, C.T.C. will e-mail the applicant directing them to the a website for processing the Student Application and the *$57 ($55 application fee + $2 service charge) application fee (via Visa or MasterCard debit/credit card).

C.T.C. will e-mail (in approximately 3 – 4 working days) both the recommending institution and the applicant to confirm the status of the credential/certificate issuance and will have the status available to view on the C.T.C. Application Status and Credentials Held look-up site. It will take approximately 10 working days from the date the on-line Student Application is processed for C.T.C. to mail the credential/certificate document directly to the applicant.

Application for Designated Subjects Paper Credential Recommendation

This process allows for the paper processing of the credential recommendation by CSUSB to C.T.C. by mail for Designated Subjects (Vocational/Career Technical Education/Adult/Special Subjects/Supervision and Coordination) credential applicants only. Preliminary credential applicants may submit the Application for Designated Subjects Paper Credential Recommendation once all required documentation is obtained. Clear credential applicants may submit the Application for Designated Subjects Paper Credential Recommendation after CSUSB grades have been posted.  GO TO APPLICATIONS & FORMS NOW

The Application for Designated Subjects Paper Credential Recommendation is initiated with the submission of a completed Application for Credential Recommendation, C.T.C. Credential Application (41-4), Required Documentation, *$55 application fee (Money Order or Cashier’s Check only) made payable to the Commission on Teacher Credentialing (or C.T.C.), and the appropriate CSUSB non-refundable processing fee (click Payment Methods for further details) in person or by mail. NOTE: The results of the program evaluation will take approximately 2 - 4 weeks from the date the application was accepted by the Credential Processing Office.

Once the Designated Subjects credential recommendation has been submitted to C.T.C., it will take approximately 3 – 6 months from date of recommendation for C.T.C. to process and mail the credential/certificate document directly to the applicant. NOTE: C.T.C. has established the Application Status and Credentials Held look-up site for the public (i.e., school districts, county offices, institutions of higher education and the applicant) to view the status of the applicant's credential/certificate application(s) processed to C.T.C., which will include the applicant's name, document number, issuance date, recommending institution, if applicable, and the available fee credit, if applicable.

Effective November 1, 2007, C.T.C. will no longer issue first-time or new-type Designated Subjects credentials entitled Vocation Education Teaching Credential. However, holders of a Preliminary and Clear Vocational Education Teaching Credential may continue to upgrade, renew and reissue their credentials after November 1, 2007.
