APPLICATION FOR FORMAL CREDENTIAL/CERTIFICATE RECOMMENDATION

Effective April 1, 2007, C.T.C. no longer accepts paper applications for formal credential/certificate recommendation (except for Designated Subjects applications). All other credential/certificate recommendations must be processed via on-line by the recommending institution.  NOTE:  Designated Subjects (Vocational/Career Technical Education/Adult/Special Subjects/Supervision and Coordination) credential applicants need to follow the Paper Application instructions below. 

The application for formal credential/certificate recommendation initiates a program evaluation and recommendation to the Commission on Teacher Credentialing (C.T.C.) for credential/certificate issuance.  The application may be submitted at the beginning of the last quarter of program course work and after verification of successful completion of all credential/certificate requirements have been submitted. 

NOTE:  Please be sure to read the instructions on the form(s) carefully and complete each applicable section.  Incomplete form(s) will be returned to the applicant for completion and resubmission.  It is highly recommended that the applicant keep a copy of all submitted form(s) for their personal records.

The results of the program evaluation will be e-mailed (via attachment) to the applicant's CSUSB e-mail address only or mailed (per request or if applicant does not have a valid CSUSB e-mail address) within approximately 2 - 4 weeks.  Please note that although CSUSB will accept the application at the beginning of the last quarter, the credential/certificate will not be formally recommended to C.T.C. until 2 - 4 weeks after CSUSB grades have been posted. Applications submitted after CSUSB grades have been posted will be evaluated and processed to C.T.C. in approximately 2 - 4 weeks from the date the application was accepted by the C.A.S.E. Office.

The recommended credential issuance date will be the official program completion date (includes course work, CPR, and/or examination(s), if applicable) -OR- if three months have passed since the official program completion date, the recommended credential issuance date will be the date the completed application was accepted by the C.A.S.E. Office.

It is the applicant’s responsibility to verify that all required original documentation is in their program file prior to or at the time of submission of the application for formal credential/certificate recommendation. Please note that all candidates must be officially and unconditionally admitted to the University and to the appropriate credential/certificate program in order to be formally recommended for a credential/certificate by CSUSB.

NOTE: Effective January 1, 2007, the Professional Clear credentials will no longer be held to the Professional Growth requirements (150 clock hours of activities and the experience requirement). Due to this change, the credential term of Professional Clear will be discontinued and the credential term of Clear will now be listed on professional level credentials.

 

ON-LINE APPLICATION FOR CREDENTIAL RECOMMENDATION

This process allows for the electronic processing of the credential/certificate recommendation by CSUSB as well as the electronic submission of the on-line Student Application to C.T.C.

The On-Line Application for Credential Recommendation is initiated with the submission of the On-Line Application for Credential Recommendation, Required Documentation and the *$25 CSUSB non-refundable processing fee (See CSUSB Payment Methods for further details). Please note that the use of this process will require a Visa or MasterCard debit/credit card to pay the C.T.C. on-line application fee and a permanent e-mail address (valid at least 3 months from the submission date of application) to receive e-mail correspondence directly from C.T.C.  NOTE:  Applicant must have a valid CSUSB e-mail address (click CSUSB Telecommunications and Network Services Web Mail for further details) in order to receive e-mail correspondence from the C.A.S.E. Office regarding the application status.  If applicant does not have a valid CSUSB e-mail address, correspondence will be mailed to the address listed on the application. 


Once the credential/certificate recommendation has been submitted to C.T.C., within a couple of days thereafter, C.T.C. will e-mail the applicant directing them to a website for processing the on-line Student Application and the *$57 ($55 application fee + $2 service charge) application fee (via Visa or MasterCard debit/credit card).  NOTE:  The on-line Student Application must be processed to C.T.C. no later than 3 months after the credential/certificate recommendation date.  If the applicant does not receive e-mail correspondence from C.T.C. within 5 working days or the applicant's e-mail address becomes invalid prior to processing the on-line Student Application, the applicant may go directly to the Student Application and follow the instructions.

Once the applicant has completed the on-line Student Application process, C.T.C. will e-mail (in approximately 3 – 4 working days) both the recommending institution and the applicant to confirm the status of the credential/certificate issuance and will have the status available to view on the C.T.C. Application Status and Credentials Held look-up site. It will take approximately 10 working days from the date the on-line Student Application is processed for C.T.C. to mail the credential/certificate document directly to the applicant.

NOTE: CSUSB cannot process renewal of credential documents via “courtesy” application electronically to C.T.C. with the on-line credential recommendation process.

PAPER APPLICATION FOR DESIGNATED SUBJECTS CREDENTIAL RECOMMENDATION

This process allows for the paper processing of the credential recommendation by CSUSB to C.T.C. by mail for Designated Subjects (Vocational/Career Technical Education/Adult/Special Subjects/Supervision and Coordination) credential applicants only.  Designated Subjects Preliminary applicants may submit the Paper Application for Credential Recommendation once all required documentation is obtained.  Designated Subjects Clear credential applicants may submit the Paper Application for Credential Recommendation after CSUSB grades have been posted.  Please note that it is anticipated, in the near future, that C.T.C. will require all applications for credentials/certificates be processed via on-line only.

NOTE: Effective November 1, 2007, C.T.C. will no longer issue first-time or new-type Designated Subjects credentials entitled Vocation Education Teaching Credential.  However, holders of a Preliminary and Clear Vocational Education Teaching Credential may continue to upgrade, renew and reissue their credentials after November 1, 2007.

The Paper Application for Credential Recommendation is initiated with the submission of a completed Application for Credential Recommendation, Credential Application (41-4), Required Documentation, *$55 application fee (Money Order or Cashier’s Check only) made payable to the Commission on Teacher Credentialing (or C.T.C.), and the appropriate CSUSB non-refundable processing fee (See CSUSB Payment Methods for further details) in person or by mail.  NOTE:  The results of the program evaluation will take approximately 2 - 4 weeks from the date the application was accepted by the C.A.S.E. Office.

Once the Designated Subjects (Vocational/Career Technical Education/Adult/Special Subjects/Supervision and Coordination) credential recommendation has been submitted to C.T.C., it will take approximately 3 – 6 months from date of recommendation for C.T.C. to process and mail the credential/certificate document directly to the applicant.

NOTE:  The C.T.C. Application Status and Credentials Held look-up site has been established for the public (i.e., school districts, county offices, institutions of higher education and the applicant) to view the status of the applicant's credential/certificate application(s) processed to C.T.C., which will include the applicant's name, document number, issuance date, recommending institution, if applicable, and the available fee credit, if applicable.
