Listed below are the credential/certificate applications and forms processed by the C.A.S.E. Office which may be required for the submission of the Application for Formal Credential Recommendation.  Please click Required Documentation for the specific documentation and forms that will be required for the formal credential/certificate application.  NOTE: Applications and forms must be submitted in person or by mail only and cannot be e-mailed or faxed. 

To view and/or print the applications and forms, Adobe® Acrobat Reader® or the Microsoft Word Viewer must be installed on your system.  Please note that all applications and forms are available in the C.A.S.E. Office and PDC Teacher Education Office for applicants who do not have printer access.
