REQUIRED DOCUMENTATION

It is the applicant’s responsibility to verify that all required original documentation is in their program file prior to or at the time of submission of the Application for Formal Credential Recommendation.  Please note that applicants who submit the application via mail, will be mailed their original examination results and CPR card, if applicalbe, after the recommendation has been processed.

NOTE: Effective January 1, 2007, the credential term of Professional Clear will be discontinued and the credential term of Clear will now be listed on professional level credentials.

To view and/or print the list of original documentation required for the submission of the Application for Formal Credential Recommendation, you must have Adobe® Acrobat Reader® or the Microsoft Word Viewer installed on your system.  Please note that all forms are available in the C.A.S.E. Office and PDC Teacher Education Office for applicants who do not have printer access.
